Dr. Dana Dameron is proud to be at the forefront of progressive

Patients interested in Medical Cannabis will make a Physician
Certification appointment with our office. Dr. Dameron will review
your medical history and documentation. He will conduct a thorough
exam and evaluate if your medical problem is approved for treatment
by applicable state laws. He will also review treatment options with
low-THC cannabis and medical marijuana, discuss risks, benefits and
alternative therapies that are available. A patient who is qualified to
receive treatment with cannabis is required by law to complete a
state-approved consent to treatment.
Dr. Dameron will review this paperwork with you and assist with the initiation of application for a Registry ID card. The state of Florida requires certain portions of the application to be completed by the patient on their own and will charge a $75 application fee to obtain the Registry ID card. As part of the application process, once the above is completed, Dr. Dameron may initiate an order in the Florida electronic ordering system and prescribe Medial Cannabis or low-THC CBD based on your individual medical needs.
Once your application is approved by the state of Florida,
dispensaries will have access to the order Dr. Dameron placed during
your visit.

Dr. Dameron will evaluate that you have attempted previous therapies
without success and that the use of Medical Marijuana for your condition outweighs the risks of therapy. Documentation of your medical condition is required for therapy. Your previous medical records will need to be directly forwarded to our office from your previous treating facilities, and you may be required to get updated documentation of your illness, such as new imaging to validate that your condition still qualifies for treatment.
The stateof Florida has outlined a list of debilitating medical conditions through State Amendment 2 that qualify for treatment with Medical Cannabis. Dr. Dameron takes the responsibility for prescribing medical marijuana very seriously and may turn away patients who do not qualify based on out of date records, missing documentation, records not forwarded directly from aprevious treating facility/physician, or failure to identify a debilitating illness that affects ability to perform regular activities
Florida list of debilitating illness as per Amendment 2 state law:
Initial visit costs $200 and includes detailed evaluation of your medical history, records review, patient exam and state certification. The state requires recertification every 7 months (30 weeks).
Follow up visits will be $100. Any changes to the original order, that differ from the dosing originally prescribed at the certification visit, will require a follow up visit. As with any medication prescribed by our office, a follow up appointment is necessary to evaluate for efficacy and determine if dose adjustment/order update is appropriate to best treat your condition. This covers additional time, paperwork and update to state registry
